Northern Counties Health Care, Inc. is a nonprofit corporation dedicated to delivering a full spectrum of quality, affordable, and compassionate health care services. Serving patients of all ages across the three-county Northeast Kingdom region of Vermont, we were established in 1976 by community residents concerned about local health care access. Our comprehensive network includes six community health centers, featuring a walk-in primary care location, three dental centers, and a Medicare-certified home care and hospice division, with our community health centers recognized by NCQA as patient-centered medical homes.
